چکیده

1 Reducing variation in total knee replacements A new study in the journal Health Affairs of total knee replacement (TKR) confirms that wide variations in care exist among hospitals based on factors such as geographic location. The High Value Healthcare Collaborative found significant variations in TKR among the 5 participating health care systems. Among these were length of stay (LOS), where patients were discharged after surgery, and complication rates (sidebar). The study also reports 3 practices for managing variability: • a multidisciplinary approach to preop preparation, used by the hospital with the lowest in-hospital complication rate • dedicated OR teams for TKRs, used in hospitals with the fastest operating times • managing patient expectations by engaging them in discharge planning even before admission, which started after the data were analyzed, has reduced LOS.

Investigation of Wear Behavior of Biopolymers for Total Knee Replacements Through Invitro Experimentation

The average life span of knee prosthesis used in Total Knee Replacement (TKR) is approximately 10 to 15 years. Literature indicates that the reasons for implant failures include wear, infection, instability, and stiffness. However, the majority of failures are due to wear and tear of the prosthesis. The most common biopolymer used in TKR  is Ultra High Molecular Weight Polyethylene (UHMWPE). Pr...

Type 3 finger length pattern is associated with total knee replacements due to osteoarthritis but not with hip replacements or hand osteoarthritis in the elderly: The AGES-Reykjavik study

BACKGROUND Recent case-control studies have shown an association between type 3 finger length pattern (longer ring finger than index finger) and knee osteoarthritis. This large cross-sectional study tests the hypothesis that the type 3 pattern is associated with total joint replacements due to osteoarthritis in a large population based study. METHODS Finger length ratios were assessed visuall...

MOBILE and the provision of total joint replacement.

Modern joint replacements have been available for 45 years, but we still do not have clear indications for these interventions, and we do not know how to optimize the outcome for patients who agree to have them done. The MOBILE programme has been investigating these issues in relation to primary total hip and knee joint replacements, using mixed methods research. There have been five main stran...

The Effect of Insert Conformity on The Wear of Total Knee Replacements

Introduction: The demand for joint replacements in young patients is increasing, and therefore there is a need to increase the lifetime of the prostheses. To fulfil this requirement, new replacements’ designs and materials are being introduced. One of the design parameters that may affect wear of total knee replacements, and hence the predicted lifetime of such prostheses, is the insert conform...

Seasonality of service provision in hip and knee surgery: A possible contributor to waiting times? A time series analysis

BACKGROUND The question of how best to reduce waiting times for health care, particularly surgical procedures such as hip and knee replacements is among the most pressing concern of the Canadian health care system. The objective of this study was to test the hypothesis that significant seasonal variation exists in the performance of hip and knee replacement surgery in the province of Ontario. ...
